The guy with the RPG is an Iraqi, he's wearing the "chocolate chip" BDU's from the 80-90's. That's what the Iraqi "Army" wears...
It's a regular RPG-7, just with a different type of projectile than you normally see.

It's the OG-7V anti-personnel round for the RPG-7. The more familiar warhead is the PG-7V HEAT (single stage warhead).
I believe it's a clarinet or possibly an oboe.
